We're sad to see you go! To make the process as easy as possible, please download and fill out the medical record release form linked below, and include the name and fax number or email of your new pediatrician if you know it. Some records are too large to be faxed in their entirety and must be emailed. 

 

We unfortunately cannot mail physical copies of medical records; however, if you would like your child's records sent directly to you, we can email them to you via an encrypted PDF. The medical record release form is required regardless-- please email the form to us when complete.
