Congratulations, you are a new parent! Once your child arrives, call and let us know so we can schedule you for a hospital followup visit 24-72 hours after you and your new baby are discharged. Please bring with you your discharge paperwork, the baby's yellow card, and the insurance card.

Please bring your child's insurance card to all appointments. Click here for a list of the insurances we accept, but it's also always a good idea to double check by calling your insurance company and asking if Dr. Joseph Richter is in network.

Below is our registration paperwork, which can either be filled out prior to the first visit and emailed to us at office@hudsonheightspediatrics.com, or filled out at the visit itself.
